Everything You’ll Need to Know
Who is eligible for voluntary benefits?

- Full time employees who work 30+ hours per week
- Part-time employees who work 17 ½ hours per week
What are the requirements to cover my spouse on the medical insurance?

- Your spouse must not have access to other medical coverage, or
- If employed, your spouse’s employer pays for less than 50% of the cost of the coverage
- If covering your spouse
- A copy of your marriage certificate must be submitted to HR at first enrollment.
- A Spouse Eligibility Form must be completed and submitted to HR every year

- The deadline to submit your marriage certificate and spousal eligibility form for this Open Enrollment is before November 19, 2025.

- Domestic partners are not eligible.
When can I enroll?

- New Hires –
- Self Enroll: within 60 days of your date of hire
- Call center enrollment assistance: 567-666-4371, Mon – Fri, 9:00am – 8:00pm EST.

How Do I Enroll?

- If you have questions about the benefits or need enrollment assistance, call US Enrollment Services at 567-666-4371 (Monday – Friday, 9am -8pm EST)
When will my benefits be effective?

- New Hire enrollments: 1st of the month, following 60 days from your date of hire. (Exceptions: Basic and Dependent Life, Short Term Disability and Long-Term Disability are effective 1st of the month following 1 year from date of hire.)
- Open Enrollment: 1/1/2026
